Care Finity accepts referrals from several sources, including:
- Physicians
- Discharge planners
- Friends, Neighbors, Family Members
- Estate Lawyers
- Hospice and Home Health Agencies
We accept VA, Medicaid, and Long Term Insurances, as well as Private Pay.
When receiving a referral, our first call is to the client’s physician for approval of an initial consultation and evaluation.
Once approved we will submit a referral to a state agency for an initial assessment. A registered nurse will then go to the client’s home and develop a plan of care specific to their needs.
The plan of care is then reviewed and approved by the physician and services are initiated.
Our registered nurses provide on-going monitoring of the delivery of care and update the plan of care as needed.
